This issue has numerous causes. Two of them off the top of my head are caused by the carbon strips that get attached to them. I wouldn't call it a major failure though. This particular panto failure didn't pull down the OCS with it.
Yes, I do realize I misinterpreted the picture, and the OCS doesn't appear damaged as it did at first.
I still feel this is a pretty serious failure though, given that the system was down for an extended period of time. Were it to occur on opening weekend, it would be a major news item.
