ION - Waterloo Region's Light Rail Transit
From a description of the incident, it's even more idiotic than I thought.

"There was no sneaking up. The train was blaring a horn and in emergency brake mode for the vehicle directly in front which had turned in front of the moving train. The train was almost stopped because of that and this vehicle turned right into the side of the train. There is not a 100' blind spot beside and behind the vehicle."
